PROBLEM-SOLVING FOR The Family Caregiver

A LABOR OF LOVE

     Our services are not only for the elder client. The role of the family caregiver is significant and critical. It can be rewarding.  It can also be overwhelming trying to navigate the complex care systems. 

     We are here to support you. We can help you customize the care you are providing to the specific needs of your loved one.

     More importantly, We are partners helping guide families, friends, and loved ones through this experience by enhancing coping skills, increasing knowledge, and helping to minimize stress.


  FAMILY CAREGIVER SERVICES CAN INCLUDE, but aren’t limited to: 


* STRATEGIES FOR adjusting to becoming a caregiver: Providing care and support to someone you love can be very rewarding.  Many adult children believe they can do it all.  Over time, the responsibilities of your own growing family, work, and the changes in your loved one's care can seem overwhelming.

  • Coping with Stressors of being a family caregiver (fatigue, limited sleep, depression and isolation, financial burden, fear and anxiety of unknown “what ifs…)
  • Learning about ‘Caring for Yourself in order to care for others’.
  • Learning how to talk to loved ones when they are resistant to receive help.        

                              

* EXPLORING OPTIONS FOR ADDITIONAL HELP for my loved one.           

  • LIAISON SERVICE: Long Distance Care Oversight and Management. We understand that you may not be with your loved one at all times and at all medical appointment. We can attend physician appointments to be listening ears for a client and family as they interpret diagnoses and treatment recommendations. We can help clarify recommendations with physicians and specialists by asking the important questions that often are difficult to think of on the spot.
  • WHEN THINGS COME UP: Crisis intervention and Transition(s). As things change for the elder client and the family caregiver, we are there as an ongoing advocate, partner, and resource; reviewing options and helping find solutions.  We can guide and support you during difficult situations.

              - Change in Condition: medical, physical, mental (dementia)

             - Hospital discharge planning, post-hospital rehab discharge  

                planning

              - Facilitate, negotiate for a new or different client placement

              - Update the care plan for additional care and support when 

                the client is at home

              - Provide information and referral to local community     

                resource experts for medical, financial, and legal advice.  


  • COORDINATE WELLNESS CHECK: When there is a caregiver team in the home, we can evaluate whether our client is receiving quality care. When needed we can provide coaching to hired caregivers in the home.
